To Qualify:

Proof of disability Title XIX or 3rd party funding Spend majority of waking hours out of bed Not dependent on alcohol or non-prescription drugs Receive approval from Iowa Foundation of Medical Care --ICF/MR level of care Require vocational or education services during day programming hours.

Licensed acute care facilities.

Ages: Services adults and children
Five community-based homes which house six residents each.
Administrative/Activity Center located in Corning, Iowa.
Community-based intermediate care program for children and adults with developmental disabilities.
